Privacy notice for website reviews

The form collects your name, email address, written feedback and an optional 1–5-star rating. Your name and email are kept private so the review can be managed and you can be contacted if needed. Email addresses are never published. Your full name or professional name is displayed only if you explicitly choose that publication option.

Choose whether your review may be published or kept private. Reviews are checked before publication. If you allow publication, choose first name only, full name or professional name, or anonymous. First-name-only reviews show only your first name; anonymous reviews show no name. Private feedback is not published.

Please do not include confidential medical records or sensitive personal health information in a public review. Your information is used for review management under this privacy policy.

Website reviews and ratings are separate from Google reviews.
